- The Telehealth Remote Monitoring Grant Program will award up to five (5) grants in the amount of $100,000 each, to eligible rural outpatient health care facilities. The grant funds will be used for implementing telehealth remote monitoring for outpatient clinical services. The intent of the grant program is to enable Colorado’s rural residents, including Health First Colorado (Colorado’s Medicaid program) members, to benefit from telehealth remote monitoring services.
